ReMind Health is a psychiatric provider group dedicated to serving geriatric patients requiring psychiatry and mental health counseling. Our practice operates in various facilities, including:
memory care & dementia units,
nursing homes & skilled nursing homes,
assisted living,
long-term care,
inpatient rehabilitation,
substance abuse & recovery, and
detoxification centers.
In addition to providing exceptional patient care, our practice recognizes the unique challenges faced by the facilities we serve. We take pride in identifying areas where we can offer assistance, including:
PDPM-aware assessments,
facility-appropriate diagnoses & indications for psychotropic medications, and
gradual dose reduction programs & documentation.
Our mission is to provide superior behavioral health services and support to patients who have transitioned, permanently or temporarily, into a facility. By employing experienced behavioral health care providers and acknowledging the distinct needs of these patients, we strive to develop a comprehensive mental health program that addresses the often challenging transition to a new reality.

All services are provided in-house, eliminating the need for patients to be transported to a clinic. This comprehensive approach includes an initial psychiatric evaluation, medication management and reviews, psychotherapy sessions with Psychiatric and Mental Health Nurse Practitioners (PMHNPs) and Licensed Professional Counselors (LPCs), and ongoing care with visits scheduled weekly to monthly, depending on the patient’s acuity and needs.
Emergency visits (tele-health) can be conducted within 24 hours, and urgent visits within 72 hours. Effective communication and coordination with patients’ primary care providers are prioritized whenever necessary. Prompt phone calls and texts from the facility are ensured. Additionally, communication with facility nurses and staff is maintained regarding medication changes and acute patient issues.
